Through strategic partnerships, Express Health Systems supports programs that extend care beyond the clinic and into the home, helping patients with complex needs live healthier, safer lives.
Choice Supportive Care Team (CSCT)
A nonprofit program providing in-home evaluations to ensure patients receive the specialty home health services they need. CSCT also offers telemedicine visits as needed, supporting both patients and their caregivers. The program works directly with home health agencies, hospice providers, and community organizations to deliver coordinated care, reduce complications, and keep patients safe at home.
Streamline Wound Care Solutions (SWCS)
A specialty wound care service that partners with home health agencies, hospice providers, and assisted living facilities. SWCS focuses on advanced wound therapies, including biologic treatments, with the goal of accelerating healing, lowering hospital readmissions, and improving quality of life for patients with persistent or complex wounds.
Both programs accept Medicare and most private insurance, ensuring patients receive the care they need with minimal barriers.
